Robotic perineal radical prostatectomy (r-PRP) offers a safe and effective minimally invasive option for prostate cancer patients, even with large prostates. This new technique preserves oncologic and functional outcomes without increasing complications.

- The patient had a large prostate volume (130 cc) with a middle lobe and a BMI of 32.
Findings:
- Robotic perineal radical prostatectomy (r-PRP) was successfully performed.
- The procedure took 140 minutes total, with 95 minutes at the console.
- Blood loss was 85 cc, and no intraoperative complications occurred.
Implications:
- r-PRP is a safe and feasible technique for patients with large prostates and prior surgical history.
- The procedure can be performed without prolonged operating times or increased morbidity.
- r-PRP shows promise in maintaining oncologic and functional outcomes in selected patients.
