Background:

Iliac branch devices (IBDs) are increasingly used for treating iliac artery aneurysms.
These devices aim to maintain blood flow to the hypogastric artery during aneurysm repair.
Successful IBD deployment is contingent upon specific patient anatomical criteria.

Observation:

The technical feasibility of using iliac branch devices is limited by strict anatomical requirements.
When these anatomical criteria are not met, standard IBDs may not be suitable for treatment.
Alternative endovascular solutions are necessary for patients unsuitable for conventional IBDs.

Findings:

Custom-made fenestrated iliac stent grafts represent a viable alternative to IBDs.
These custom grafts can be utilized when patient anatomy precludes the use of standard IBDs.
This highlights the adaptability of endovascular treatment strategies for complex iliac aneurysms.

Implications:

The availability of custom fenestrated grafts expands treatment options for complex iliac aneurysms.
This enhances the ability of vascular surgeons to manage challenging anatomies endovascularly.
Improved patient outcomes are anticipated through tailored endovascular solutions for iliac artery disease.
