Background:

  • Nuclear factor-kappa B (NF-κB) is a key regulator of immune responses and inflammation.
  • Osteotomy and bone implant procedures can induce significant surgical stress and inflammatory reactions.

Purpose of the Study:

  • To evaluate the inflammatory response, specifically NF-κB activation, following osteotomy and implant placement in rat femora.
  • To assess the impact of surgical maneuvers, including irrigation and biomaterial application, on modulating this inflammatory response.

Main Methods:

  • Wistar rats underwent surgical procedures: bone perforation (control), implant insertion, intense irrigation, or application of a vitaminic compound.
  • NF-κB activation was quantified using immunohistochemistry six hours post-surgery.
  • Osteocyte NF-κB levels were determined by direct counting of labeled and unlabeled cells.

Main Results:

  • The control group (bone perforation without irrigation) exhibited the highest NF-κB positive cell count (33.4 ± 2.45).
  • Implant placement without irrigation showed reduced NF-κB levels (28.9 ± 2.70).
  • Intense irrigation (24.3 ± 2.72) and vitaminic compound application (26.5 ± 2.60) significantly decreased NF-κB positive cells compared to the control.

Conclusions:

  • Surgical techniques during osteotomy significantly influence post-operative inflammation.
  • Adequate irrigation or the use of specific biomaterials can effectively minimize NF-κB activation and control inflammation during bone surgery.
