Background:

  • Cherubism is a rare, benign autosomal dominant disorder causing progressive, painless enlargement of the jawbones due to fibrotic tissue and giant cells.
  • Lesions typically stabilize and regress after puberty, but severe familial cases can persist.

Observation:

  • A 14-year-old male presented with severe familial cherubism, experiencing significant mandibular enlargement since age 4.
  • The enlargement impacted speech and mastication, showing no signs of regression by puberty.

Findings:

  • Surgical intervention involving composite mandibulectomy and fibula free flap reconstruction successfully improved both jaw functionality and facial aesthetics.
  • Histological examination confirmed cherubism, revealing fibrotic tissue and giant cells. Notably, unique basophilic laminated concretions were observed in regional lymph nodes, a previously unreported finding.

Implications:

  • This case highlights the potential for cherubism to manifest severely and persist beyond puberty, necessitating surgical intervention.
  • The novel lymph node findings warrant further investigation into their pathogenesis and potential diagnostic significance in cherubism.
