    Background:

    • Extra-short implants offer a potential alternative in limited bone scenarios.

    Purpose of the Study:

    • To evaluate radiographic and clinical outcomes of extra-short implants.
    • To compare outcomes with regular-sized implants in the posterior maxilla.
    • To assess the efficacy of osteotome sinus floor elevation with extra-short implants.

    Main Methods:

    • Systemically healthy non-smokers with posterior maxillary tooth gaps were included.
    • Implants were placed based on residual bone height: extra-short with sinus lift (<4mm), extra-short alone (4-7mm), or regular (≥8mm).
    • Radiographic measurements (crestal bone level, residual bone height) and clinical outcomes were assessed at baseline, 6, and 18 months postloading.

    Main Results:

    • Implant survival rates varied: 96.3% for extra-short, 95.8% for regular, and 93.1% for extra-short with sinus lift.
    • Crestal bone level was significantly higher with regular implants versus extra-short with sinus lift at 18 months (P < .028).
    • Crestal bone level change was significantly lower with extra-short implants + sinus lift compared to regular implants between 6 and 18 months (P = .003).

    Conclusions:

    • Extra-short implants, with or without osteotome sinus floor elevation, yield comparable clinical and radiographic outcomes to regular implants.
    • These techniques represent promising, non-invasive treatment options for the posterior atrophic maxilla.
    • Implant/prosthetic characteristics may not significantly influence short-term crestal bone level changes.
