This pilot study shows that objective peer comparison metrics can be implemented in emergency medicine residency programs. These metrics offer a potential objective addition to the current subjective milestone evaluation system.
Area of Science:
- Medical Education
- Quality Improvement
- Patient Safety
Background:
- Current Emergency Medicine (EM) resident evaluations via The Milestones are subjective and imperfect.
- There is a need for objective metrics to assess resident competency in patient safety and quality improvement.
Purpose of the Study:
- To evaluate the implementation of an objective peer comparison system for EM residents.
- To quantify aspects of quality, safety, efficiency, throughput, and utilization.
Main Methods:
- A pilot study was conducted at an academic, tertiary care center.
- Metrics were compared within postgraduate year (PGY) classes using Wilcoxon signed-rank and rank-order analyses.
Main Results:
- Significant changes were observed in most metrics across all PGY classes.
- PGY3s showed significant changes in EKG and X-ray reads; PGY1s and PGY2s in disposition to final note share.
- Physician evaluation to disposition decision did not reach significance in any class.
Conclusions:
- Objective metrics can be successfully implemented in EM residency training.
- Peer comparison metrics may serve as an objective supplement to the existing milestone evaluation system.

Renal Drug Clearance: Comparison Between Renal Excretion Methods
Renal clearance is often associated with the renal glomerular filtration rate (GFR), which represents the rate at which plasma is filtered through the glomeruli in the kidney. When drug reabsorption is minimal and there is no active secretion, renal clearance is closely related to the...
