Compact Bone01:27

Compact Bone

Most bones contain compact and spongy osseous tissue, but their distribution and concentration vary based on the bone's overall function.
Compact bone, also called cortical bone, is the denser, stronger of the two types of bone tissue. It is found under the periosteum and in the diaphyses of long bones, where it provides support and protection. The microscopic structural unit of compact bone is called an osteon, or haversian system. Each osteon is composed of concentric rings of calcified...
Bone Marrow Sampling and Transplants01:22

Bone Marrow Sampling and Transplants

Bone marrow transplant is a potential cure for several diseases, including cancer and specific genetic disorders. Notably, this procedure is applicable for patients suffering from aplastic anemia, certain types of leukemia, severe combined immunodeficiency disease (SCID), Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma, thalassemia, sickle-cell disease, and certain cancers.
The transplant begins with high doses of chemotherapy and radiation treatment, which aim to destroy the...

Area of Science:

  • Oral and Maxillofacial Surgery
  • Dental Implantology
  • Bone Grafting Techniques

Background:

  • Autogenous bone grafts from the mandibular ramus are used for alveolar ridge augmentation.
  • Conventional harvesting methods risk bone marrow invasion, leading to complications like pain, swelling, and nerve injury.

Purpose of the Study:

  • To present a novel, complication-free harvesting technique for autogenous bone from the mandibular ramus.
  • To evaluate the outcomes of bone grafting and donor site regeneration using this new method.

Main Methods:

  • A grid-type cortical bone harvesting technique was employed, involving ditching holes and micro-saw osteotomies to create cortical squares.
  • Harvesting was performed from the occlusal aspect, with extended osteotomies to prevent bone marrow invasion.
  • The harvested bone was used for dental implants in one patient.

Main Results:

  • The patient experienced no severe postoperative pain, swelling, or numbness.
  • After 15 months, the donor site showed new cortical bone formation.
  • The grafted area achieved cortico-cancellous healing, supporting functional implant loading.

Conclusions:

  • The grid-type cortical bone harvesting technique effectively prevents bone marrow invasion.
  • This method allows for successful autogenous bone grafting for dental implants with predictable donor site regeneration.
