Background:
- Peripheral osteomas (PO) are rare, benign, slow-growing jaw lesions, typically solitary and unilateral.
- Multiple POs without syndromic associations like Gardner syndrome are exceptionally uncommon.
Observation:
- A 75-year-old male developed multiple, 2.0 cm peripheral osteomas in the buccal bone adjacent to dental implants across three posterior sextants over 7 years.
- These lesions were clinically and radiologically presumed to be peripheral osteomas and surgically excised due to difficulties with oral hygiene and peri-implant health maintenance.
Findings:
- Histopathology confirmed the masses as peripheral osteomas, both compact and cancellous.

- Surgical resection led to significant improvements in chewing function and oral self-cleansing.
