A rare case of pyelocaliceal diverticulum containing 107 stones was diagnosed in a man with microscopic hematuria. The kidney stones were composed of calcium oxalate, calcium phosphate, and urate.

    Area of Science:

    • Nephrology
    • Urology
    • Pathology

    Background:

    • Microscopic hematuria prompted investigation in a 68-year-old male.
    • Initial imaging suggested renal stones, but their location was atypical.

    Observation:

    • Abdominal plain film revealed numerous calcifications in the left renal region.
    • Excretory pyelogram demonstrated stones separate from the main collecting system.
    • Partial nephrectomy successfully removed 107 stones.

    Findings:

    • Histological examination confirmed pyelocaliceal diverticulum as the underlying condition.
    • Infrared spectrophotometry identified stones composed of calcium oxalate and calcium phosphate.
    • X-ray microanalysis detected magnesium, and SEM revealed a core of urate and calcium oxalate with concentric laminations.

    Implications:

    • This case highlights the importance of considering pyelocaliceal diverticulum in cases of unexplained renal calculi.
    • Understanding the complex composition of these stones aids in diagnosis and management.
    • Further research into the pathogenesis of stone formation within diverticula is warranted.
