Inflammatory Bowel Disease V: Surgical Management01:21

Inflammatory Bowel Disease V: Surgical Management

123
Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Esophageal Strictures-II: Clinical Features and Management

51
Patients with esophageal strictures often experience a range of symptoms. Initially, they may have difficulty swallowing solid foods, which can progress to include liquids. Additional symptoms may involve chest pain or discomfort, regurgitating food and fluids, heartburn, unintentional weight loss, coughing or choking during meals, and hoarseness.
Healthcare providers should gather a comprehensive medical history and conduct a physical examination for diagnosis. If esophageal stricture is...

Esophageal Varices-II: Clinical Features and Management

46
Esophageal varices often manifest as gastrointestinal bleeding episodes, presenting symptoms like hematemesis (vomiting of blood), hematochezia (passing fresh blood via the rectum), and melena (black, tarry stools). Other signs can include weight loss, anorexia, abdominal discomfort, jaundice, pruritus, altered mental status, and muscle cramps.
In the initial assessment, a thorough review of the patient's medical history is vital to identify risk factors such as liver disease, alcohol...

Larynx

1.3K
The human larynx, often referred to as the voice box, is an intricate organ located in the neck. It serves as a pathway for air to enter the lungs during respiration and is an essential component of voice production.
Anatomy of the Larynx
The larynx consists of various components, including cartilage, muscles, and vocal cords. Its structure includes three large unpaired cartilages—the thyroid, cricoid, and epiglottis—and three smaller paired cartilages—the arytenoids,...

Barrett Esophagus-II: Clinical Manifestations and Management

111
Individuals with Barrett's esophagus are often asymptomatic, but they may experience symptoms commonly associated with GERD, such as heartburn and acid regurgitation. Additional symptoms can include difficulty swallowing, chest pain, unintentional weight loss, blood in the stool (which may appear black, tarry, or bloody), and episodes of vomiting.
To diagnose Barrett's esophagus, healthcare providers often recommend an endoscopy for those showing symptoms of acid reflux. The procedure...

Summary

Sulcus vocalis causes hoarseness and vocal fatigue. Microsurgery improved vocal parameters like MPT, but some dysphonia persisted, indicating incomplete voice restoration.

Area of Science:

  • Otolaryngology
  • Speech and Language Pathology
  • Vocal Health

Background:

  • Sulcus vocalis is a vocal fold abnormality affecting voice quality.
  • It often presents with symptoms like hoarseness and vocal fatigue.
  • Understanding its clinical and acoustic characteristics is crucial for effective management.

Purpose of the Study:

  • To analyze clinical, vocal, and videolaryngoscopic parameters in patients with sulcus vocalis.
  • To evaluate the effectiveness of microsurgery in treating sulcus vocalis.
  • To compare vocal parameters before and after surgical intervention.

Main Methods:

  • Observational study of 60 patients diagnosed with sulcus vocalis (type II or III).
  • Data collected included demographics, vocal symptoms, videolaryngostroboscopy, perceptual and acoustic vocal analyses, and maximum phonation time (MPT).
  • Vocal parameters of 22 patients who underwent microsurgery were analyzed pre- and post-operatively.

Main Results:

  • Hoarseness (100%) and vocal fatigue (88%) were primary symptoms.
  • Type II sulcus was most common (90%), often associated with polyps (23%).
  • Microsurgery reduced GRBAS scores and SPI, increased MPT, but did not normalize all acoustic parameters.

Conclusions:

  • Sulcus vocalis leads to mild to moderate dysphonia, characterized by roughness, breathiness, and elevated fundamental frequency.
  • Surgical treatment offers improvement in vocal parameters.
  • Despite surgical benefits, complete restoration of vocal quality remains a challenge.
