Background:

Reoperative parathyroidectomy is more complex than initial procedures.
Limited data exist on how index cervical operations impact reoperative outcomes.

Purpose of the Study:

To characterize reoperative parathyroidectomy outcomes based on the index cervical operation.
To identify specific risks associated with different prior surgical histories.

Main Methods:

Retrospective review of 242 adult patients undergoing reoperative parathyroidectomy for primary hyperparathyroidism (2016-2024).
Stratification by index procedure: focused parathyroidectomy, subtotal parathyroidectomy, total thyroidectomy, hemithyroidectomy, and nonendocrine anterior cervical procedures.

Patients with prior subtotal parathyroidectomy face higher surgical failure risks and more frequent autotransplantation.
High-volume centers, thorough preoperative risk assessment, and a low threshold for autotransplantation are recommended for reoperative parathyroidectomy.
