Abstract:
The absence of standardization in the donation after circulatory death (DCD) organ donation process is well known across the transplant community. According to Scientific Registry of Transplant Recipients (SRTR) data, there were 1629 unique hospitals that performed ≥1 DCD organ donation in 2024. Given the large number of hospitals performing DCD organ recoveries, it is not surprising that standardization remains challenging. LifeQuest Organ Recovery Services (FLUF) has partnered with Mayo Clinic, Florida, since 2023 to develop a hospital-based donor care unit to centralize both DCD and donation after brain death organ recoveries from surrounding areas. By centralizing the DCD donor process, one can ensure that DCD protocols are standardized and aligned with best medical practices. Herein, we describe key components of our standardized DCD model, including protocol development, training, family support, withdrawal-of-care procedures, operating room logistics, perfusion technologies, and pathology practices. We also highlight the issues that must be considered and the barriers that must be overcome when centralizing DCD donation. The hospital-based donor care unit model exemplifies how centralized recovery can optimize both donor management and organ outcomes, and it should serve as a national framework for best practice implementation for DCD organ recovery across the United States.
