Combined liver-small bowel transplantation is a lifesaving option for children with intestinal failure and end-stage liver disease. Careful patient selection, guided by residual bowel length, enteral tolerance, and liver fibrosis, is critical. Advances in surgical techniques, immunosuppression, and perioperative care have improved survival rates. Long-term outcomes rely on multidisciplinary management, growth and neurodevelopmental monitoring, and vigilant surveillance for infection and rejection. Emerging strategies, including bioengineered grafts and optimized perfusion, hold promise for further improving outcomes in this complex population.
