Purpose of the Study:

  • To determine the proportion of adults with RA who underutilize rehabilitation.
  • To identify demographic and clinical factors associated with rehabilitation underutilization in RA.

Main Methods:

  • A cross-sectional online survey was administered to 309 adults with RA receiving care at an academic medical center.
  • Rehabilitation underutilization was defined as moderate-severe functional limitation combined with no self-reported use of RA-related rehabilitation services.
  • Associations were examined with variables including race, ethnicity, insurance status, financial status, education, work status, and BMI.

Main Results:

  • 25% of participants with RA underutilized rehabilitation services.
  • Moderate-to-severe functional limitations were reported by 47% of participants, while 63% had never used RA-related rehabilitation.
  • Underutilization was strongly associated with having Medicaid and Medicare, no insurance, lower financial and education status, disabled work status, obesity, and major comorbidities.

Conclusions:

  • A significant proportion of adults with RA underutilize essential rehabilitation services.
  • Individuals with lower socioeconomic status and greater functional needs represent a population with a clear unmet need for rehabilitation.
  • Developing targeted strategies is crucial to improve access and utilization of rehabilitation for adults with RA.
