Evaluation of Lead Properties Affecting Complete Radiological Success in Transvenous Lead Extraction

Background:
The insertion of cardiac implantable electronic devices has become increasingly common, resulting in a growing need for lead extraction, most often due to infection and malfunction. The majority of the research indicated that the lead indwell time was the most important factor in determining the procedural success of extraction. In this study, we examined the factors affecting complete radiological success.
Methods:
A total of 192 lead extractions were performed on 102 patients at Başakşehir Çam and Sakura City Hospital. These procedures were compared based on lead indwell time and lead properties, considering the complete radiological success.
Results:
Complete radiological success was determined by lead indwell time and the presence of ICD leads. For leads with an indwell time of less than 12 years, the complete radiological success increased more than fivefold. Furthermore, in patients with ICD leads and lead indwell time exceeding 12 years, more complex and additional extraction techniques may be beneficial rather than simple traction alone.
Conclusion:
Prolonged lead indwell time and the presence of ICD leads were identified as two key factors affecting both complete radiological success and procedural difficulty.
