Combined bronchogenic and dermoid cyst is an extremely rare entity, with only three previously documented cases localized to the back and oral cavity. We herein present the first case arising within the clinically concealed perirenal space. A large perirenal cyst was incidentally detected on imaging in a young female patient and preoperatively considered as teratoma. Complete surgical excision was performed, and the final histopathological examination confirmed the mixed cystic lesion. This case highlights that urologists should include this rare combined lesion in the differential diagnosis of fat-containing perirenal masses besides teratoma.
