VELscope Fluorescence Method for Early Diagnosis of Oral Potentially Malignant Lesions - A Systematic Review and

Objective:
To evaluate diagnostic ability of velscope for various oral potentially malignant disorders (OPMDs).
Method:
Review adhered to Preferred Reporting Items for Systematic Reviews and Meta-Analysis - Diagnostic Test Accuracy (PRISMA- DTA) guidelines. Databases were searched till April 2025 to identify the diagnostic potential of velscope. Quality of selected studies was evaluated based on QUADAS (Quality assessment of diagnostic accuracy studies)- 2 tool. Pooled sensitivity, specificity, likelihood ratio (LR), diagnostic odds ratio (DOR) and area under curve (AUC) was performed in Meta-Disc 1. software and Review Manager 5.3.
Results:
Nine studies were included in review and for meta-analysis. Included studies evaluated the diagnostic accuracy of velscope for various OPMDs like oral leukoplakia (OL), oral lichen planus (OLP), oral submucous fibrosis (OSMF) and oral candidiasis in comparison with reference standard like toluidine blue test (TBT) and biopsy followed by histopathological investigation. Meta-analysis showed pooled sensitivity and specificity of 0.73 and 0.65 for OL, 0.70 and 0.80 for OLP, 0.72 and 0.84 for OSMF, 0.68 and 0.80 for candidiasis while an overall AUC of 0.75, 0.81, 0.80 and 0.50 was seen for OL, OLP, OSMF and candidiasis respectively.
Conclusion:
Velscope is a promising and valid tool and overall has good diagnostic potentiality in diagnosing the target condition and can be used as an alternative adjunct to histopathology. It can be undertaken for early diagnosis and prompt treatment under secondary level of prevention.
