    Summary

    Localized bullous pemphigoid (BP) may occur without detectable basement membrane zone (BMZ) antibodies or complement. Autoantibodies in BP might be a disease marker, not the cause, suggesting new diagnostic approaches.

    Area of Science:

    Background:

    • Bullous pemphigoid (BP) is an autoimmune blistering disease.
    • Immune deposits (immunoglobulin and complement) at the basement membrane zone (BMZ) are typically detected in BP patients.

    Observation:

    • Two cases of localized BP were observed without detectable antibody or complement at the BMZ.
    • Evidence suggests immunoglobulin and complement at the BMZ may be a laboratory marker, not etiologically significant.

    Findings:

    • BP serum failed to induce blistering in organ culture.
    • High incidence of immune complexes in BP patients.
    • Spontaneous autoantibody increase observed in the elderly.

    Implications:

    • Autoantibody production in BP might serve as a disease marker without direct causative role.
    • Re-evaluation of the pathogenic role of autoantibodies in BP is warranted.
    • Potential for non-immunological diagnostic or therapeutic strategies in localized BP.
