Eliminating friendly fire. Successful nursing documentation strategies

Often, an institution's documentation policies, procedures, and records become frustrating instruments of torture rather than helpful resources for the nurses who must use them. In this article, the authors highlight several of the strategies used by one nursing service department to eliminate barriers to adequate documentation.
