    Background:

    • Entrustable Professional Activities (EPAs) are crucial for competency-based assessment in medical training.

    Purpose of the Study:

    • To identify EPAs for Internal Medicine (IM) Educational Milestones.
    • To operationalize competency-based assessment for residents using EPAs.

    Main Methods:

    • Modified Delphi approach with a 2-step cross-sectional survey of IM educators and residents.
    • Content validity indices and independent sample t tests were used to analyze data.
    • Participants rated the importance and training year for 30 proposed IM EPAs.

    Main Results:

    • Seventeen EPAs achieved a 100% content validity index; 10 more exceeded 80%.
    • Educators and residents showed agreement on the importance of 27 out of 30 EPAs.
    • Residents perceived 10 EPAs could be achieved one year earlier than educators suggested.

    Conclusions:

    • Internal medicine educators maintained stable opinions regarding the developed EPAs.
    • Residents generally agreed with the identified EPAs, supporting their utility.
    • The study provides a foundational list of EPAs for resident evaluation in IM programs.
