Background:

  • Verrucous carcinoma is a rare, highly differentiated squamous cell carcinoma known for local aggressiveness and slow growth.
  • It has a minimal potential for metastasis, but can invade underlying tissues.

Observation:

  • A 48-year-old male presented with a hyperkeratotic, ulcerated, wart-like lesion on the distal phalanx of his left third finger.
  • The lesion, present for 8 months and repeatedly traumatized, showed rapid evolution with edema and erythema.
  • Histopathology confirmed verrucous carcinoma, with X-ray revealing osteolysis of the distal epiphysis.

Findings:

  • The clinical presentation and histopathological examination confirmed verrucous carcinoma.
  • Radiographic evidence indicated invasion into the bone of the distal phalanx.
  • Surgical amputation of the distal phalanx was performed due to the advanced nature and acral localization of the tumor.

Implications:

  • This case highlights the importance of considering verrucous carcinoma in acral lesions, especially with a history of trauma.
  • Early diagnosis through clinical and histopathological correlation is crucial.
  • Surgical excision, including amputation for advanced acral cases, remains the treatment of choice due to invasiveness and recurrence risk.
