Abstract:
Home care nursing provision has evolved significantly over the last twenty years to meet the needs of the population and adapt to the constraints of the health system. Following in the wake of healthcare networks, network centres and territorial support platforms, new tools have been designed to support home care nurses in coordinating the care of people with complex health needs.
