Intramedullary spinal cord tumors are rare yet highly morbid. Contemporary neuropathology uses integrated diagnosis-histology plus immunohistochemistry, DNA/RNA sequencing, and DNA methylation profiling-to define tumor type. In this article, an overview is provided for tumor entities that occur in the spinal cord in adults and children. For each tumor type, the epidemiology, histologic characteristics, hallmark molecular alterations, prognosis, controversies, and clinical care points are summarized. This review aims to provide a quick reference for neuropathology diagnosis to accelerate therapy selection and highlight areas for future endeavors.
